Turning off Call Waiting
By far the easiest way to disable Call Waiting for one call only is simply to place *44 for Telstra lines, or #87, (comma included) for Optus lines, in front of your ISP's dial up number. This will turn off Call Waiting for that call only. Then you don't have to remember to turn it off and on again every time.

Alternatively the option in Windows Control Panel under "Telephony" can do this, provided the area code without "0" prefix, (for International dialling) and "Use area code and Dialling Properties" is selected in the Dialler properties (see below).

Note that with this setup the 613 prefix will be omitted from the dialling string but the *44 will always be included.
